I think it's called any time a scoring chance has been diminished in any way. According to NHL.com:

24.1 Penalty Shot – A penalty shot is designed to restore a scoring opportunity which was lost as a result of a foul being committed by the offending team, based on the parameters set out in these rules.

So no, it's not as simple as hook on a breakaway=penalty shot.

I though he regained control and got off a full shot; i.e. a reasonable chance to score
