I’ve never met one of those. I’ve dealt with numerous folks with PhDs but every one has said to use his or her first name.
Related, are dentists true doctors?

I can't be the only one that checks the "dr" prefix on random forms, right? Not on anything official but I always laugh that it's even an option, so have some Sliver type fun with it

I call my dentist Dr and it’s mostly out of courtesy I suppose. What do you say when you call and make an appointment for example?

Since I feel dentistry should be included in medical healthcare I don't oppose calling a dentist Dr.

I had a Sports Illustrated subscription with "Dr." as my prefix. I was 8.
