On the Flyers goal, a miscommunication between Gaudreau and Ferland, where Gaudreau followed the Flyer's D across to Ferland's side of the ice, and Ferland drifted that way left the Flyer's winger with a huge amount of space on the wall.
Giordano leaves the front of the net (leaving Hamilton with 2 Flyers to deal with) to close off the space the puck-carrier has. Ferland, having also seen the Flyer with a huge amount of space, closes as well and both Ferland and Gio get stuck in no man's land around the hash marks.
Monahan follows his man to the net, but doesn't manage to tie up his stick, goal against.
I'm not exactly sure who made the first error between Gaudreau and Ferland, my guess is Ferland, but this goal is a good example of the very simple fact that, in the NHL the players are all so good that almost any mistake is likely to end up in the back of the net.

A guy is loose with the puck on the offensive zone, so someone should go put pressure on him. That's the right play for one player. But clearly there's confusion who should do that, so Monahan, Ferland and Giordano all move that way. I'd also say it's pretty clear from their body language none of them decisively know it's his job, because none of them go do it in a decisive way.
That leaves Hamilton alone in front of the net.
It doesn't make the coach look good if such simple play makes his players lose track of who should be doing what.
