KOTOR was an amazing game, I just remember it being pretty buggy.

2 was the first Mass Effect game I tried and I loved it. Tried 1 and it was pretty good. Tried 3 and quit playing after about 1/2 an hour, haven't got back to it yet. Part 2 was perfect combat mechanics which made it a lot of fun to play.

Baldur's Gate 2
I'd also include the original X-Com but a few tweaks could have improved it a lot. The atmosphere was amazing on that game though.

The multiplayer portion of Mass Effect 3 is perfect.
KotOR as mentioned is a total classic, but I think a remake could actually improve the game. Even the sequel has some great mechanics that could be adapted into it, for example, squad mate alignment. Making dark side a little more nuanced would be interesting as well, it really just feels like there’s no consequence for being nice, so being a bad guy just boils down to being a dick and hurting others for fun, rather than out of necessity or because going dark is the easier path. Granted, I’ve never had it in me to play through dark side, so I might just be missing out.

The consequence is that the game was way harder.

Disagree, master force speed + force valor + dual sabres was pretty much a cakewalk. Been a long time but most enemies would fall in 1 or 2 turns. The dark side powers were pretty much useless except fear but stun had pretty much the same effect as fear. Stun droid was overpowered as well.

From a wargame simulation perspective Harpoon 2 was nearly perfect. It was a blend of tactics and understanding technology, and it worked extremely well.
There was nothing worse then doing a convoy escort and suddenly missiles start popping up from every direction. The enemy was really smart. They'd use subs and airforce and completely bush wack you.
I remember one mission where I had to bomb airfields in I think it was Iraq. I sent my carrier task force up and decided I needed a way to get rid of the Iranian Airforce. So I launched a strike of F-14 escorts and F-18 fighter bombers. But I turned off the radars of the F-14's and left the F-18's radiating. I approached the coast line and a huge number of Iranians fighters launched. So I turned off the F-18 radars and sent them home and activated the F-14's and sent them in launching my phoenix missiles from 100 miles away. It was a cool bushwack and I cleared the skies and sent my bombers in. Meanwhile I had to keep an eye on my fleet because I kept picking up sub signatures.

Some old school picks...
Earthbound on SNES. I am not into RPG games at all, but that one has had me hooked since 1995. I will still play through it every year or two to bring back the memories.
Tetris on NES. Such a classic. In addition to just seeing how high you can get your lines, you also have that mid to late game portion where you have to decide if you want to keep going for tetrises or just play it safe with singles.
Super Mario World for SNES. I like this one most of the old school mario games. I feel like it was the right balance of complexity and simplicity.
Mario Kart for SNES. This was the ultimate battle game between friends for our neighborhood. Whether it was doing a cup series or battle mode, the intensity was always high.

Super Mario 3
Assassin's Creed II
Uncharted series, particularly 4.
You might even add Duck Hunt to the list.
It's hard to separate perfect games from favorite games.

For sure. I had a list of about 10 typed out but then thought about it for a few minutes and 80% of them still had something I wish they did better. Like GTAV, so close to perfect but the way rockstar hates mods and only supports online now prevented that making the list

I'm not sure there have been any. The Last Of Us is probably the closest thing to. I get why people are saying things like FFVI, and I love that game, but the writing in it is often juvenile and there are far too many party characters. Chrono Trigger has probably just as good a case and has similar writing, because, hey, JRPGs, amirite? ME2 would have a good argument for it, if it weren't the middle game of a series and therefore informed by a flawed predecessor and successor that it can't reasonably be separated from. Same is true of Uncharted 2.
I think video games are a medium that weren't, until quite recently, equipped to deliver a "perfect" experience, and recently the incentives haven't been there to permit it - those who have the resources to make a game perfect will intentionally make it worse to make more money.

Three words my friend
Metal Gear Solid
First 3, I can't think of a single flaw. 4th was awesome too but too many cutscreens. 5 was fun but got bland about halfway in

Man, that stealth in MGS2 was... frustrating at times. I guess it's possible I just sucked at the game, but I did ultimately beat it. I just remember a whooooole lot of "wtf" moments with that obnoxious "!" Sound effect.

DOTA as a Warcraft 3 custom game.
Essentially spawned the entire MOBA genre - LoL, DOTA 2, HotS - and likely the largest esports segment. The original wasn't at all dissimilar to the iterations today, and had a loyal fanbase from 2003 to 2013 when dota 2 arrived on Steam. I played it that whole time!

If we're judging the earliest perfect games.
Asteroids and Pacman would be considered perfect games.
