I know of 2 pretty horrific workplace accidents involving contractors I have worked with.
One was a drilling accident. There were 3 guys working on a small rig. The consultant and one of the drillers went into the truck for lunch and the other driller started operating the auger (no one knows why - he wasn't supposed to be working still). When the other 2 guys were finished lunch, they started walking towards the rig and heard thumping noices. When they got to the rig, they saw the other driller mangled in the auger, which was still spinning and his head was thumping off the side of the machine. Needless to say, he died. They had an emergency shutoff cord , but apparently it wasn't working. Now, whenever I work with drillers, I always get them to test it in front of me.
The other accident was a guy doing cleaning at a factory using an industrial pressure washer. He dropped the hose and it was kicking around so fiercely that it impaled him from his hip and right out his neck.
