Go to the Ontario website. you can enter all the required information and pay online.

I did this a couple years ago and had my new Birth Certificate pretty quickly. It was way easier than I had expected.

I filled out the form from the gov't website, printed it and dropped it off at Harry Hays. That was Feb.10.2009, and received the new passport Feb.19.
