I don't remember which documentary I saw and which tribe it focused on but one of the main reasons they can survive in isolation is because they are free of foreign disease. They have developed to be able to combat what occurs on their island but any foreigner who sets foot could be bringing all sorts of disease that will wipe them out.

I read about one island where most of the inhabitants were wiped out after an outsider brought disease. Visitors were banned after that.
