I doubt it's the engineer. This is on someone who was supposed to check the routing with Transportation; usually, this task is on a transportation contractor or, likely, on a clerk at the tracking company...

I’d be willing to bet that this is a span/abutment interface issue with mounting points not lining up, rather than the bridge span itself being 10-12cm +/-. If it’s an interface issue, it will fall to PCL and their fabricator to fight over money. If the bridge is actually the wrong length and matches the latest rev of approved drawings, then it will be PCL and the city fighting about it.
Hopefully it’s a mounting issue and not an overall length issue. Fixing mount points could be as easy as coring new anchor bolts.

Somebody messed up but this stuff happens all the time, though usually on a smaller scale. Heck just a couple weeks ago we upgraded a major part of our plant with a new part from the oem manufacturer and even a consultant to help us. A major peice for whatever reason wasn't even close to fitting and required some major modifying to make it work. Haha still funny.

I don't understand why the city doesn't just do the Calatrava Bridge for every single overpass they build. They paid for the design. Imagine, red finger trap bridges everywhere.

It's impossible. The private sector is perfect. 
(On a more honest note, I have some not great memories of finger pointing on problems with contractors on most jobs I've worked on, some more noticeable to the public than others!)

Must have been a slow news day.
The construction nusiness is a constant battle to make things fit that don't fit.
In this case, very likely a surveying issue, or a measurement issue. The steel arch likely had to be ordered before a site measurement could be taken.
The fix is likely chipping some bearing plates out, moving them, and casting them back in. The big cost is the cost of moving the crane in a second time.
Shouldn't have anything to do with the city.
