as best as i can tell the punchline is simply that the baby didn't die. but this is exactly what makes this comic so good, having a few that you can't understand makes it much more intriguing. Far Side was the same way

It's also done as a woodcut, which is pretty awesome. You don't see that often in webcomics (or regular comics for that matter)
There was a version of that one where instead of a baby almost falling into a river, the carriage was filled with bottles instead, and a bum shaking his fist angrily at death.
It's a great comic for sure. Astronaut Fall and No Survivors are favorites of mine.
