I haven't but I have dealt with their customer service department.
It was exceptional.
In short, give them a call and ask. They are good questions and I am sure they would answer them for you.

Not sure about all products but I tried ordering tart cherry juice from them and they didn't deliver to Canada.

I had no problems, my kegerator was delivered to my doorstep in 4 days. Bonus, I was able to pay with Visa!

Yes, this should all work. We have ordered many things to our place in the states, and I am 90% sure we just used our Canadian credit cards.

I bought some stuff online and compared to in house prices it was a ripoff. Online and warehouse are apparently two separate entities, and ordering online being a member makes no difference. If you can get it in the warehouse, you're better off.

The higher online price reflects the shipping cost to home. Often the product will say something along the lines of "this item may be available in warehouse at a cash and carry price". This is best seen with the TVs and mattresses. It seems the US site doesn't include shipping, though?
The online member price is the listed price. Non-members pay 10% premium I think.

We've order at Costco.com before and had the deliveries made to a shipping/receiving company in Montana because they don't ship to Canada. We haven't tried lately but apparently about 2 years ago Costco.com started blacklisting the shipping/receiving companies along the border and would cancel orders which were being delivered to such places.
I don't recall whether or not they accepted Canadian credit cards but I have one of my credit cards set up with a secondary billing address in the USA so that I can use that with American retailers who can not process Canadian billing addresses.

Good point. Like I said, I am pretty sure they did take our Canadian credit card with our Canadian billing address, but if not you can just call your credit card company and add a secondary US billing address temporarily.

We use Montana Shipping Outlet in both Sweetgrass and Eureka. They can forward things to Canada once it arrives but shipping is generally not cheap into Canada and there is always a chance it gets tagged for tax and duty by the border folks.
We always just drive down and pick up our stuff but we usually go down 4 times a year. We spend 2 or 3 nights on average and that way we get the full import exemption.

I use Package Express for forwarding. Pretty standard shipping rates.

They have always been able to forward to Canada. You just have to give them a call once your package arrives, tell them to forward it to your address in Canada and provide a credit card number for payment.

Odd. When I signed up I thought they noted on the site that they didn't offer forwarding, which I why I signed up for Package Express in the first place.

That might be the case now because it may have become too much of a hassle for them to forward things on to Canada. From my previous experience, they did provide forwarding although it was not something they advertised. You just had to call in and ask for it to be forwarded. I suspect that they probably got pissed off at people always asking if they could undervalue the shipment or declare it is a "gift" on the customs paperwork in order to try to avoid taxes and duty.
For forwarding they only used UPS as well which was terrible because of the high brokerage fees they charge.
