Just a note about the survey. I think the wording of the questions will lead to most people selecting yes, regardless of if they would be willing to actually use an app to order. This might mean that your results won't be very useful, unless you just want to convince someone that lots of people want this, like an investor or potential restaurants.
Adding addition questions about what keeps people from using a mobile option, if they would use a mobile option to simply replace phone ordering or what features they would like in such a service would actually be better.
Right now, you are making a single choice for the respondents, which I can't see anyone saying no to, because of they way it is worded. Of course I would want a discount if I used mobile ordering. Of course I would want to carry those credits over for as long as possible. You don't account for other possibilities, like saving a reoccuring order, having an order placed in advance or a free additional side or drink with your order, for example.
To give you some useful info: I am one of the most technically inclined people I know. If there is a gadget to do something with, I likely own it, or want to own it. If there is some sort of web service to do something, I use it. I order items online on pretty much a weekly basis, from my phone or PC. I can pay with Paypal, Bitcoin and whatever credit card with minimal effort by using my phone. If there is an app for something, I use it.
Despite all this, I have never any sort of online ordering after my first attempt with Papa John's a couple years ago. The reason? I felt like I wasn't getting the best deal possible, in that I was missing out on a special that I normally ordered. If I call the store, I make sure that I clarify that there isn't some kind of new special on. You make it easy enough to do that on an app, and I am there, and once tech people adopt it, the others follow.
