Risk Matrix

Ok,

So I am sitting at work and have been asked to put together a risk matrix for a report that i am putting together.

Does anybody know what the hell a risk matrix is and how I would put one together????

I if Remember right its a risk versus failure chart. Where you have the situation and actions on seperate verticles, and where they meet is the consequences

All I can suggest is a Google search. It appears though that CaptainCrunch has the right idea. There are many pictures as well if you click the "images" button.
