I'm pretty sure a woman's body can just reject a fetus w/out notice. So she feels a pain, runs to the bathroom... blood comes out. She may or may not know what has actually happened or inspect the blood. Flushes toilet. Fetus ends up in water purification. At least that is my theory.

You are correct. I would guess it is a teenage girl who doesn't know she was pregnant or doesn't realize she has lost the fetus.
