A signed consent form is useless at the best of times, and getting someone to sign while knowingly impaired would invalidate any 'informed consent' from the get go. He could easily be charged with battery/assault etc.

Not that hard, actually. She only has to prove it on a balance of probabilities, and if the court believes her evidence that's probably enough.

Even then, alcohol doesn't always vitiate consent. However, it's definitely arguable that since she wanted Narnia and was unaware that she was signing over artist's discretion, and since he knowingly and maliciously slipped it to her while drunk and with full knowledge that she didnt read the changes, the contract is void under "unclean hands" and non est factum.
Even then, people generally can't consent to disfigurement... and thats what a pile of poo is.

Quite a few tattoo artists film the consent and the actual tattooing just in case of buyer's remorse.

From a medical-legal standpoint, those consent forms often do mean shart. I can't imagine tattoo parlours are held to a higher standard.
