Boy this one's tough. Couple runner-ups are Santa Monica by Everclear and On My Way by Rusted Root. But I think the winner is:
If I Should Fall From Grace With God - The Pogues.
Definitely my favorite song at the moment. Though, if I had to die at the songs end as you indicated i'd be pretty inclined to make 'As Slow As Possible' (639 year track length) my new all time favorite song ...

Mine would be Paranoid Android by Radiohead

Great pick. My favorite band of all time. I would also have to pick one of their tunes, but it's so hard to choose "Just" 1. I think I might also pick Paranoid Android, or else Let Down or Creep.

Mine changes once in awhile. For a very long time it was AC/DC's For Those About To Rock (there aren't many songs that can compare live)
But I'd have to go with Travelling Riverside Blues, as done by Led Zeppelin

Dark Angel by Blue Rodeo
Hurt by Johnny Cash would be an extremely close runner up.

Often underappreciated...
That one was almost a b-side on MCIS. It's sister song "Medellia of Grey Skies" ended up there instead.
For me...

Garden by Pearl Jam.
Not because it's my favourite song, but because it has sentimental value and brings back good memories.
Hard question to answer though. I seriously had to think about it.

For some reason Superstition by Stevie Wonder jumps out. Pretty up lifting in my final hours and about believing in things I don't understand. Which you'd have to assume you might be doing at that point.

Something by Radiohead. Depends on the mood I guess... Fake Plastic Trees probably. Potentially We Suck Young Blood.
I'm not sure what kind of mood I'll be in if this is right before I die... do people like upbeat music at that point?
