I'm guessing that you are talking about if there is blood, it turns to 4 minutes.

The rule is that it is the discretion of the refs, as to whether the injury will lead to a disruption in the players game. Usually blood means that they could miss a shift and therefore the ruling is for 4 minutes. This doesn't always happen. Last night Pavelski was highsticked an had blood on his lip. The refs looked at it and still ruled 2 minutes.
