Medvedev has a strong, reliable second serve and can vary his game with more than just a single tactic if things aren't working. The fact he is a bit of a wild card like you say, works in his advantage too.
I can honestly not pick out anything that Zverev has improved on in the last 3-4 years aside from hitting a few more aces with a frantic first serve. His second serve is arguably the worst on tour. Such a waste considering he's got all the physical tools to be just as good as Medvedev.
