I've used Teksavvy for a couple of years. It's pretty good. Their customer service is better than the big companies (it would be hard to be worse) and they're cheaper. Speed generally reflects what they advertise.

A handful of times the service has dropped out (from a couple of hours to a few days at the longest). In those cases, they have to rely on the provider they're effectively subletting capacity from (in my case, Shaw) to actually fix it, so you're still dependent on the whims of those horrible companies to some degree. I'm guessing the rate of downtime doesn't significantly exceed what it would be if I were just paying Shaw directly, though, so all in all I'm happy with them.
