In the case of Ping and Titleist this is included in the price. Not sure about TM or Callaway.

I know that for TaylorMade, lie, loft and length adjustments are free of charge and there would only be an upcharge if the customer wanted a shaft that carried an upcharge. I would be surprised if Callaway, Mizuno, Cobra, et al. would charge for a lie, loft or length adjustment. Most custom orders take about two weeks to receive.
