Like in your example, why stop with vaccine refusal? How about things like smoking or obesity? Those are things that cause many many deaths with clearly defined science showing the links to health issues.
Because smoking and overeating are often caused by addiction and mental health issues, and don't really affect others. Every person who refuses a vaccine has a societal impact that affects not only themselves, but also the vulnerable. One person can inflict healthcare costs on a multitude of others, along with illness causing lost employment hours and efficiencies. It has a far greater multiplying affect than a dude smoking in an alley or eating too many cheeseburgers. Being fat isn't contagious.
