Like in your example, why stop with vaccine refusal? How about things like smoking or obesity? Those are things that cause many many deaths with clearly defined science showing the links to health issues.

Probably because things like addiction and obesity are multitudes more complicated than a preventative vaccine.
Let's revist this silly comparison when I can recieve a thoroughly researched and scientifically proven injection that effectively and efficiently reduces or even eliminates those health risks with minimal adverse side effects.
