The official reason for Goose's death was aircraft mechanical error - the parachute deploying while the canopy was still above Goose. Although, some argue Goose mis-deployed the ejection before the canopy was clear.
It could largely be attributed to the F-14's mechanical systems and jet engine placement which cause an engine flameout and the flat spin. The F14 being susceptible to flat spins due to asymmetric air flow and the canopy not floating away if in a flat spin.
In reality, the engine flameout was caused by turbulent airflow, which, in turn, was caused by being too close to the exhaust of the jet in front of them... which was Iceman:
