Last week my wife ran into a parked car on our cul de sac and did about $3000 worth of damages to the back end of it of a 1993 Ford Taurus. The car is not worth more than the damage it incurred so it is a write off.

We have decided to give the driver a $1000 payout of the 1993 vehicle and he accepted. In addition I am going to replace his tail light.

However, I want to get something in writing.

I cant find any "contract" type forms on the internet and was wondering if anyone has experience with this? What should I include in said document.
