I don't work in Healthcare so this idea might not work.
I think when you go to the ER when they triage you they should then send you either to the ER or a walkin clinic staffed by Nurse Practitioners. If you're diverted to the walkin you're taken first come, first serve.
That would divert the hang nails, minor ailments away from the ER, and people would then have an idea of the wait time as they'd go in order.
It reduces pressure on the ER Docs, and reframes expectations on the patient.
They would need different waiting rooms, but I really don't see the downside.
