I think the Formenton issue is pretty straight-forward (assuming that he was in fact the ringleader).

When this originally came out, it is entirely possible that the victim knew one name (but not the rest), as she would have met that person at the bar (or wherever), prior to the events unfolding.
The events would have taken place in one hotel room (that of the ringleader) - and the victim may have known the room number, as she was in fact invited there.
Even if the victim didn't state the room number, it could well have been reported somewhere (even by an outsider, such as a pizza delivery man) - and then easy enough for someone to dig up who's room it was.

There are a myriad of possible reasons why one name could have been reported (or at least mentioned) during the early proceedings, and the first time this all came out. So it is entirely possible, maybe even probable, that Ottawa knew specifically that Formenton was directly involved (without any other names being released or directly discussed).
