I'll start this off by saying that someone giving the impression of consent because they are under duress or feel threatened, obviously isn't consent and doesn't change whether an assault occurred.

Regarding pizza guy though, if he walked into a situation where something like this is happening, it isn't unreasonable that he didn't know how the situation unfolded and if there was duress. He could have been confused or just assumed it was consensual. Some people literally are that dumb, especially if they don't have a lot of life experience.

There's that It's Always Sunny sketch where Dennis wanted to get a boat to take girls out on the ocean and have sex. Mack questions him about it saying, what if they don't want to have sex, and Dennis said that of course "no means no", but they aren't going to say no, because of the implication of danger. Mack is confused and realizes that it sounds like rape to put someone in a position where they are forced to not say no. It's meant to be dark comedy of course, but it is a pretty good illustration of how some guys will manipulate situations to try and make it look like there is consent when there really isn't.
