Not necessarily, it can be due to saturation of active ingredient to carrier, (eg: 10mg/ml of carrier versus 20mg/ml of carrier) a higher saturation can cause a greater immune response which will result in more pain at the site. Different types of carrier fluid such as bacteriostatic water, and various oil and solvent combinations can be used depending on the particular medication. Certain people react differently to what carrier oil is used.

Fluid volume on its own isn't necessarily a reliable indicator.
