I feel that GMs are often afraid of losing trades. For a player like Ohtani, it's impossible to get 'fair value'. So looking back 5-10 years from now, a GM would rather have been brave enough to go for it and get nothing, rather than having a reputation of throwing in the towel and getting not enough back. I agree with you, it's terrible and selfish mismanagement.
