I didn't for one second think that he was being too dramatic or overblown in that scene, or at any time during the movie.

I read Fast Food Nation over 3 years ago (might be closer to 4 now...), and have eaten at a hamburger joint only once, during a moment of weakness, at Harvey's in Edmonton. I had eaten (and still continue every once in a while) at Subway or Quizno's a few times, but I hadn't eaten authentic greasy fast food in about 16 months at that point. Not only that, but I had dramatically changed my eating habits, staying almost entirely away from processed foods, eating four or five times the amount of fruits and vegetables as I used to, taking supplements/vitamins, etc. I've even been on three juice fasts, trying to rid my body of all the toxins I've ingested over the years. I'm somewhat lazy, and although I do get plenty of exercise, I don't follow a scheduled routine and go to the gym in spurts. In other words, eating healthily was a easy way for me to take better care of myself.

Anyways, back to Harvey's. I chowed down, and felt nauseous, bloated, gassy, and lethargic the entire way back to RD. I didn't quite feel like vomiting, but the feeling was awful. If I was eating three of those meals a day, I would be hurling out the side of car in the drive-thru lane also, no doubt about it.
