I don't think I've ever heard mention of Tommy John surgery other than in reference to a baseball player getting it. So, if you don't follow baseball, I could see it being something you haven't heard of (or heard of enough to come to you in that short time).
Even calling it Lou Gehrig's Disease seems less common now, replaced mostly by ALS. I wouldn't be surprised if a lot of people under 40 have never heard it referred to as Lou Gehrig's Disease.
