For me it was probably playing hockey although I did not realize it at the time. Just some pain in my arm when gripping my stick otherwise nothing. What pushed it over the edge was scrolling with my computer mouse at work, and a poor ergonomic setup where my arm was not in a comfortable position at my work station. Oh yeah, and apparently a gradual weakening of the tendon as the years have gone by (getting old.....
