Shoulders can be a tricky thing. My guess is a partial thickness somewhere in the rotator cuff. Would be enough to cause pain and mobility/strength issues, but not enough to totally disable.
I always assumed that the players were given some say themselves. He probably had the choice between playing and missing 3+ months, and chose to play.
