Really bothers me. A lot. There is such an incredible focus on materials selection for engineers, techs, maintenance etc in every aspect of machinery especially those directly affecting people. To me this represents multiple massive failures that surely multiple people were complicit and CHOSE not to say or do anything to rectify the problem. Another case of “somebody” else’s problem, not mine. Combined with “that’s how stuff is done around here”.

I think it was a bad materials handling system that led to a human error.
When someone grabs the part from the warehouse to make it you have the highest risk point as it’s never tested again. So this company appeared to have all identical looking rods of the same diameter of steel beside each other and a person grabbed the wrong one and fed it into the machine.
Segragate different materials in fab shops. To me this is a system problem less than a negligence or human one
