I think feet can be tricky.
I have a similar experience too. First doctors thought maybe it was gout, then a bone infection, but finally a bone scan found it was broken bone that x-rays couldn't see for whatever reason.
This was 6-7 years ago, and it was finally at the foothills hospital that a doctor sorted it all out and ordered the tests. My family doctor and the podiatrist I went to weren't curious enough to get to the bottom of it, or maybe they gave up first? I hate to say it like that but that's how it seemed at the time.
They had me in moon boots, an aircast ( I can't remember if those are the same thing, but I had two different ones) And finally they had to build a custom shoe for me because every time I did anything It would re-break. That might be why they want you in a plaster cast?
