People sometimes call me by my last name, more times than normal but when they do that they spell it right so it doesn't bug me. It's simple respect, we had someone named Christopher, called him Chris once he said he didn't like that so addressed him as Christopher from then on.
Edit: And just to point out its not a hard name its 4 letters . . .

Totally agree. Always address people by their names and get it right.
For example, I learned this in meetings that people don't like to be called, you guys, or these guys. We had a 20 something guy in a meeting keep referring to two older ladies as these guys. "Once I finish my part I'm going to pass it over to these two (pointing) and you guys can look over it." It did not go over well.
Just with a bit of awareness he could have said, "I'm going to take a few days to complete my section and It would be great if Julie and Sandy can have a look over it and finalize it. And if there are any issues, I'd be happy to work together to complete it."
