An actress who literally wears wigs for a living having to *oh no* optionally wear a wig. I get it, it's a medical condition, but there are medical conditions, and then there are medical conditions - you know what I mean? Not to mention the decades of it being perfectly acceptable to make fun of bald men (condition or not).
Personally I think it's more about her not being able to capitalize on her condition if she allows people to make fun of it. Interviews, appearances, benefits, "face of alopecia" etc etc.
