Whether Chris Rock knew about the alopecia or was making fun of what he thought was a bad hairstyle, it was a tasteless joke. Yes, I know other comedians do it. I don't find them funny either.
Will Smith walking up on stage and hitting him was wrong. He made it all about him. If he wanted to "have his wife's back" (which married couples should do), he could have said something in his speech about Rock being jealous because he had the most beautiful wife in Hollywood, or something like that.
I think it was real. What benefit does anyone get from staging this? Certainly not Will Smith, because instead of talking about his Oscar, people are talking about what an idiot he is. I doubt that's the image he wanted to convey when he's up for best actor.
When Chris Rock saw him coming onstage, he probably was expecting something along the lines of Kanye/Taylor Swift moment - Will Smith grabbing the microphone and telling him off.
