I guess he was referring to Bieksa having the Ryan Kesler body issue in frame, which Bieksa does to poke fun at Kesler?
If you translate "guy with his tarp off" to mean "naked man", then the line is effectively, "if you have a photo of a naked man you're definitely positive for something" which sounds a lot like calling someone gay, and then tying that to being HIV positive.

possibly be what he meant.
