He's an incorporeal lifeform; Q can take on whatever appearance he wants. That he ever appeared as a middle-aged human man was a conceit he just happened to go with, to make it easier for the puny humans to interact with him. He often materialized himself in a Starfleet captain's uniform, imitating Picard's; there's no reason to believe Q wouldn't "age himself up" just for the fun of imitating Picard again.
