In this case that ref was looking to make a call against the Flames and did. Simple as that. It was soft as hell, and he knows it.

I have definitely seen penalties called that on replay were proven to be incorrect, such as high sticking penalties when a player was hit with the puck or a teammate's stick. And there was the ref (not knowing that his microphone was live) who admitted to his colleagues the other night that he made a pretty borderline call because he wanted the team to be penalized. Refs are human and do make mistakes and assumptions at times, just like the rest of us.
Edit: just saw your response above, I agree it is usually more that they think they have seen something that didn't actually happen.
