If the hospitals start overflowing the number will ramp up but it’s currently very low compared to the recent sustained positive test rate.
Yes actually attributing cause of death is a complex process and every region does it differently. I agree with your list but in particular in Florida, there is a lot more dying ‘with COVID’ than ‘from COVID’. And absolutely no presumed cases. The Florida government made these changes about a month back and are now particularly relevant after they took over as COVID epicentre
