They're both horrifying, but context is important.
Given the nature of Ramsey Bolton, rape should have been expected by everyone watching this character over the years. It's a little disingenuous to clutch your pearls after watching him torture Theon for so long and destroy him as a human being.
It's not the norm besides some of the Targaryens to mate with your cousins, let alone brother and sister. Since it's considered far outside the norms of the culture, it should be viewed as pretty horrible. It was so bad that it caused the catalyst for this entire show when it was accidentally viewed by Bran.
