Sweden charges a small fee for going to the doctor ($25). So does Norway. Those are two of the most egalitarian countries in the world. And both countries rank higher than Canada in health care outcomes.

Unnecessary visits to doctors and to ER are actually a big expense. There are people who go to the doctor every time they and or their kids have a cold. ERs have frequent visitors, often lonely people or hypochondriacs who go to the ER multiple times a month, often on a schedule ("it's Friday night Nancy"). It's estimated one in five visits to the ER in Canada are unnecessary.
