Body of work. It's really not as difficult as you make out. In this particular instance, I agree you probably shouldn't give him a card, but on the other hand, it might have a salutary effect on embellishment if they started punishing the obvious fakers.
As far as it being a judgement call, well, the refs make judgement calls all the time. That's what they're paid for.

Clattenburg just said that Layun should have been red carded IF VAR had been consulted (which it wasn't) for a deliberate stamp and violent conduct.
That bit is clear. But to start taking an arbitrary decision on whether or not to card Neymar based on an individual subjective opinion of their level of hurt is just silly.
