I think it's clear the guy on the show did not get the reference, just as it's clear JohnnyFlame missed it as well. It had exactly nothing to do with any comparison to Bush/Hitler, and just meant she was in disbelief about what Limbaugh was actually saying, i.e. Michael J Fox was faking his symtoms. In reality, if you want to say she was comparing anyone to Hitler, it was herself and her reaction.
Really, it's pretty funny that a relatively clear exchange, even if you don't get the exact pop culture reference (which I, like probably 90% of people had to look up), can be distorted to such an extent. It probably highlights a big failing of the left in media relations - the Limbaugh model of loud, absurd accusations repeated daily is much more successful with the masses than obscure references and nuanced points.
