The airlines have doctors on the ground, and then consult with their dispatch people and what not and they tell them where to go. Pilot obviously has the final say, but I remember working one when I was controlling and he was closer to Edmonton and it was clearly the better choice but he said their company said go to Calgary so... off they went.
