The trips are pretty much all-inclusive but you want money to buy snacks, bottled water, beer/alcohol, meals outside the standard program, tickets to special events such as the Mass Games etc. There are also lots of souvenirs to buy if you are so inclined. Generally, the souvenirs are the type you put away when you get home and then 2 years later when you clean out your closet you wonder why the hell you ever bought it.
Getting North Korea won seems hit and miss. It's available if you put in the effort. Some people do get it, others not. Euro is the main currency for tourists but RMB and USD are generally accepted.
