There was injury on the play, the opponent was defenseless.

a. Because he recovered.
b. Because there wasn't really that much force. Henderson just wasn't expecting the hit and was off-balance. Players who are gliding but run into each other get badly hurt all the time because they can't brace for it.
c. Did you even see how he got up? He hardly made it to his feet and was bent over half way to the bench.
"Injury on the play"? This isn't a play related hearing. Opponent defenceless? There was no opponent.
