Fib fractures come in all shapes and sizes. It is absolutely possible to sustain one and not have it hurt much (tiny avulsions or a high fib or incomplete oblique types). If it needs to go to surgery, I would have my money on Bouma's injury not being one of these types.
To further cloud the issue, it is also possible to have a non-displaced fracture with a blown syndesmosis. These are rare and usually involve an undiagnosed fracture elsewhere (tib plafond).
