Food is more expensive in remote areas. Most workers are also blue collar, and do shift work. Every small northern town has a Subway, an A&W, and if they are lucky, a Tim's. Healthy options aren't always desirable or available.

There is also probably a demographic brain drain from the country to the cities that has some sort of correlation.
